Sworn declaration

This is a sworn statement under the provisions of Article 239 of the Uruguayan Penal Code - Ideological falsification by an individual - "Whoever, due to the granting or formalization of a public document, before a public official, makes a false statement about their identity or state, or any other factual circumstance, will be punished with 3 to 24 months in prison".

Likewise, I declare that I have all the necessary documentation to prove the information included in this declaration, which may be requested from me by the Uruguayan authorities at any time during my stay in the territory of the Republic of Uruguay.

Informed consent clause

"In accordance with Law No. 18,331, of August 11, 2008, on the Protection of Personal Data and Habeas Data Action (LPDP), the data provided by you will be incorporated into a database, which will be processed exclusively for the following purpose: Authorization of Entry to Uruguay.

Personal data will be treated with the appropriate degree of protection, taking the necessary security measures to avoid its alteration, loss, treatment or unauthorized access by third parties that can use it for different purposes for which they have been requested from the user.

The person in charge of the database is the Ministry of Public Health of the Republic of Uruguay and the address where you can exercise the rights of access, rectification, updating, inclusion or deletion, is 18 de Julio, 1892, Montevideo, as established in the LPDP".
